Open Question: What do I write on a consent form for serving food? What are laws in South Carolina?

I do not have a business license but I buy, cook, and serve food for a lot of small parties in their home. Usually 10 to 40 people. They know I do not have a business license but I don’t wont someone to sue me for food poisoning or something.

What are the laws for this and what kind of consent forms should I type up?
